Mainline Benefits

Eligibility for employee benefits is as follows, If hired on the 1st day of the month, benefits begin that day. If hired after the 1st day of the month, benefits begin the 1st day of the month following the hire date.

Medical Insurance

  • All Mainline employees will be provided with individual health insurance paid in full by the company; dependent coverage is available and is employee paid. The Blue Cross Blue Shield Summary of Benefits, available from the Human Resources Department, explains the coverage for the PPO.

Dental Insurance

  • All Mainline employees will be provided with individual dental insurance paid in full by the company; dependent coverage is available at a cost to the employee (see 2Bbelow). Our dental insurance provider is Meritain.

401k Plan

  • An employee is eligible to participate in the 401k plan at the beginning of the month following employment.
  • Mainline will match employee contributions, up to 2% of their annual compensation (maximum of $4,400), and employees will be fully vested after 3 years of employment.

Life Insurance

  • Life Insurance equal to previous year W-2 earnings (to maximum of $500,000) of the employee is provided by the company. Salary is used as a basis of benefit for employees who have not worked a full year.
  • A base benefit of $60,000 will be used for employees earning commission only.
  • Please note: Employees receiving life insurance coverage in excess of $50,000 are subject to income tax on the excess benefit amount each year. This tax is deducted biweekly from the employees pay.
  • Employee has the option to purchase additional life insurance on self and/or dependents. May choose from Universal or Term Life. If employee enrolls within the first 30 days of employment, insurance is guaranteed issue. If employee enrolls after first 30 days of employment, will have to prove insurability.

Disability Insurance

  • Long Term Disability Insurance is provided through Unum Life Insurance.
  • Each employee is entitled to up to 60% of his/her monthly salary (maximum of $10,000) payable after 90 days for as long as disabled.

Vacation/Sick Leave

  • Full-time employees will accrue vacation leave based on their years of service. Less than five years of service -2 weeks per year /Five to nine years of service - 3 weeks per year Ten years of service - 4 weeks per year
  • Full-time employees will accrue 48 hours of sick leave per year.

UNUM Supplemental Insurance

  • Mainline does not pay for supplemental insurance, but it is available to you. The policies available through UNUM are: Life, Accident, Cancer, Personal Care & Short-term Disability.

Medical and Dependent Care

  • The flexible spending account (FSA) enables you to redirect a portion of your salary to provide reimbursement for two specific types of expenses: dependent care and un-reimbursed medical. Up to $3,500 for medical and $5,000 for dependent care can be deducted, pre-tax, to pay for these expenses (see Flex One handbook for details of qualified expenses).

Long Term Care Insurance

  • Mainline provides employees with the basic plan at no cost to the employee, dependent coverage is available and is employee paid. $1000.00 per month benefit, $36,000.00 lifetime maximum, 100% paid for facility care, 50% paid professional home care, Option to buy up additional coverage, Guaranteed issure for first 30 days of employment for employees, and Option to cover most family members.
